Visits from home

See your care team by phone or video. If technology isn't your favorite thing, that's fine — a plain phone call works, and we walk you through everything else, step by step.

Your care comes from an interdisciplinary team — clinicians who actually talk to each other about you, rather than a different stranger at every visit.

A coordinator who knows you

Every patient has a dedicated care coordinator: one person whose job is keeping every part of your care on the same page, so nothing falls through the cracks.

Atlantic Telehealth is part of US-LTN — the United States Local Triage Network: daily health monitoring, lifestyle medicine, and local clinical teams for patients managing chronic disease. US-LTN is a program of Rappahannock Health Corporation, a 501(c)(3) non-profit organization (EIN 86-3528712), and a CMS ACCESS pilot participant.

Day to day, Atlantic Telehealth works with Rappahannock Health in Washington, Virginia — the network's southern clinical hub and the command center for its daily telemetry.

Andrew F. Kohler, MD, FACP, MBA, CPE

President, Atlantic Telehealth

Dr. Kohler is board-certified in Internal Medicine and a Fellow of the American College of Physicians, and holds an executive MBA from the University of Virginia's Darden School of Business. He leads clinical work across the US-LTN network.
